Monitoring of chronic diseases
Monitoring of chronic diseases

If you have a chronic disease, Uniprix-affiliated pharmacists can help you monitor your condition. They work together with other health professionals (e.g. treating or prescribing physician), to keep your file up to date and make sure your treatment is working for you.

Anticoagulant therapy

Anticoagulant therapy is a treatment used to thin the blood in order to prevent the formation of blood clots and embolisms.

Certain patients with specific conditions or illnesses may require anticoagulant therapy (e.g. CoumadinTM [warfarin]) to reduce the blood coagulation and thus prevent serious complications like infarctions or blood clots.

Our Uniprix-affiliated pharmacists provide two services to ensure that the anticoagulant agent taken by patients is adequate. These are:

  • INR blood tests
  • Dosage adjustment

Ask your affiliated pharmacist for an anticoagulant therapy health status log.

Cholesterol

Essential to the human body, cholesterol is a fat-like substance produced mainly by the liver, but also taken from the food we eat. Too much cholesterol in the blood can lead to problems.

People with elevated levels of bad cholesterol risk developing various health problems. Your cholesterol must be closely monitored to avoid complications associated with elevated levels. Uniprix-affiliated pharmacies provide services to help you track this condition:

  • Cholesterol level measurement
  • Consultation for cholesterol follow-up

Ask your affiliated pharmacist for a cholesterol health status log.

Hypertension

Commonly referred to as “high blood pressure,” hypertension is characterized by elevated arterial pressure in the blood vessels.

While most people who suffer from hypertension do not experience any symptoms, this condition can nevertheless lead to certain complications such as strokes and heart attacks. We offer many services to help you keep a close watch on your blood pressure:

Ask your affiliated pharmacist for a hypertension health status log.

Diabetes

Diabetes is a chronic disease that prevents the body from producing enough insulin or using insulin effectively. Insulin is a hormone produced by the pancreas and is essential for turning glucose (sugar) in the food we eat into energy.

Left untreated or poorly managed, diabetes can lead to serious consequences and various complications, such as heart disease and kidney failure. That’s why Uniprix-affiliated pharmacists offer the following services:

  • Glycemia measurement (blood sugar)
  • Consultation for diabetes follow-up

Ask your affiliated pharmacist for a diabetes health status log.
